Forme...a spin on the word "form" but also a complimentary meaning of "for me." At Forme, we form effective programming to achieve the health goals you wish to attain. We talk about form with exercise, forming effective coping strategies and healthy lifestyle habits, 

I'm Lindsey, or better known as "Lj",  and I am the owner & founder of Forme Physical Therapy, LLC. I am a Doctor of Physical Therapy, fitness instructor and mat Pilates instructor. I have been practicing Physical Therapy for 9 years and have 10+ years experience in the fitness industry

I am an advocate of how the right type and right amount of movement can have remarkable impacts on a person's pain and function. ​


I started Forme because over my years of practice I was witness to too many of my clients limited by the restrictions of the healthcare system and insurance limitations. Additionally, I wanted to provide my patients with a more whole body approach to their recovery, as it becomes more and more apparent that an injury to one area doesn't isolate itself to just one place, and in-network care (working with insurances) limited my ability as a provider to do so, thus Forme was born. Often patients come to me having experienced a domino effect of their health (physically and psychologically), resulting in pain in multiple parts of their bodies, increased stress levels, altered and inefficient breath patterns, heightened nervous system activity and more.  I've worked with too many clients whom were suffering because they had little to no direction as to where to even begin living an active lifestyle again after pain experiences. Many coming off of months/years of pain, limiting injuries, and/or fear of what an active lifestyle might do to their pain, I found there was a missing link between Physical Therapy and re-entering the world of activity/exercise beyond the day to day demands and their "home exercise program.". While very important and valuable, many clients wanted to do more than learn how to get out of a chair, climb stairs, lift their laundry, etc. They had bigger goals and ambitions, like hike with their spouses, play baseball or soccer with children/grandchildren, go to the gym, or return to running. The thing was, they didn't know where to start, and the healthcare system limited what could be done through Physical Therapy. They felt lost once they met PT goals, despite how much education I provided, they needed and deserved the guided instruction. Insert, Forme. 


I witnessed clients spend thousands of dollars on physical therapy, only to return to or attempt to begin an exercise program and be back on my table the next month. My PT services were only scratching the surface of the underlying needs of my clients and their bodies. And through coached fitness and my Pilates practice, I began to find clients get better and stay better longer after Physical Therapy. ​


My mission is to help those who are eager to listen to and learn what their body is capable of, reduce and prevent unnecessary pain and establish sustainable health and wellness .
